RESUMO

Introduction : Lip squamous cell carcinoma is a malignant lesion of aggressive behavior, which must be recognized by health professionals to prevent damage to patient's health.Objective:To present incisional biopsy importance as an effective clinical approach for the diagnosis of lip squamous cell carcinoma and actinic cheilitis malignancy as well as the professional's lack of knowledge on these two diseases. Case report:The patient was under medical treatment due to be positive for human immunodeficiency virus and because of the actinic cheilitis on lower lip. Because the patient did not observe the labial lesion regression, he searched for the School of Dentistry of the University Center of the Educational Foundation of Barretos, where he underwent biopsy. This procedure confirmed the diagnosis of lip squamous cell carcinoma. The patient was referred for treatment in a specialized cancer center and has been living with the cancer consequences. Conclusion:The physician and dentist must be aware of the main clinical features of lip squamous cell carcinoma so that they can establish its correct diagnosis and early treatment.

RESUMO

O carcinoma espinocelular é a neoplasia maligna mais comum da cavidade bucal. Acomete com mais frequência homens com idade em torno dos 60 anos, podendo estar associada ao tabaco e elitismo. Trata-se de uma doença com alto grau de morbidade e mortalidade. O prognóstico para o paciente depende muito do estadiamento em que a lesão encontra-se, de tal sorte que, quanto mais precoce for o diagnóstico, maiores serão as chances de qualidade de vida para o paciente. No presente relato, os autores apresentam um caso clínico de paciente sexagenário, masculino, fumante de cigarro e etilista. Paciente estava em tratamento médico - histoplasmose. No exame clínico observou-se uma lesão ulcerada localizava-se na região de assoalho de boca, dolorosa, avermelhada e sangrante ao toque. O diagnóstico diferencial foi carcinoma espinocelular, histoplasmose e paracoccidioidomicose. Após a biópsia incisional e citologia esfoliativa constatou-se que o paciente apresentava um carcinoma espinocelular invasivo. O paciente foi encaminhado para um centro de referência em câncer de Barretos, São Paulo. O prognóstico foi desfavorável ao paciente. Este artigo discute a importância da biópsia, o diagnóstico tardio e a necessidade da melhor competência profissional frente a casos de carcinoma espinocelular.


The squamous cell carcinoma is lhe evil neoplasm more common of the buccal cavity. It attacks with more frequency men with age arond the 60 years, could be associated to the tobacco and alcoholism. It is treated of a disease with high morbidity degree and mortality. The prognostic for the patient depends a lot on the staging in that the lesion is, of such a luck that, the more precocious it goes the diagnosis, larger they will be the chances of life quality for the patient. In the present report, the authors present a clinical case ofpatient with 60 years old, masculine, cigarette smoker and alcoholic. patient was in medical treatmet - histoplasmosis. In the clinic examination it was observed a ulcerated lesion located in the area of mouth floor, painful, red and hemorrhage and paracoccidiodomycosis. After the biopsy and exfoliative cytology it was verified that the patient presented a squamous cell carcinoma invasible. The patient was directed for a reference center in cancer of Barretos, São Paulo. The prognostic was unfavorable to the patient. This article discusses the importance of the biopsy, the late diagnosis and the need of the best competence professional front to cases of squamous cell carcinoma.
